In response to the pandemic, we have made many changes to keep both our patients and our staff safe. To that end, we ask that you observe the following guidelines:
You must bring your own mask or face covering and keep it on at all times in our office.
You may not bring anyone with you into our office, except that obstetrical ultrasound patients, minors, and patients who do not speak English may be accompanied by one family member.
To avoid having to transfer forms back and forth in our office, we ask that you complete all necessary forms at home. The forms are available here. For forms that have an electronic version, using them would be much appreciated.
You must fill out the Screening for Possible COVID-19 Exposure form within 24 hours of your appointment. If the answer to any of the questions is Yes, please call us to reschedule.
If you are late for your appointment, please understand that we will have to cancel your appointment if there won't be enough time to disinfect rooms between patients.
Upon arrival at our office, please proceed to the front desk check-in at your scheduled appointment time.
